Safety is of utmost importance in any industry. Implications of not following safety can be disastrous. Accidents can cause not only financial loss but also can cause loss of equipment, production downtime, loss of life, loss of reputation. To prevent the recurrence of incidents and accidents, proper investigation of incidents and accidents is necessary to find the root causes. An accident investigation is also a legal requirement.
This training course is intended to provide the participants the knowledge about why the incidents/accidents happen, how to investigate the incidents/accidents and find out the root causes of them. The training course is intended to emphasize upon participants that incident/accident investigation is necessary to prevent similar incidents/accidents in future or at some place in the same organization and also to fulfill the legal requirements.

Module 2 - Interviewing Witnesses and Gathering Information
- Preparation for the interview of witnesses and other concerned persons
- Principles of effective interviewing
- Separation of witnesses

- Collecting factual information – photographs, sketches, videos, system alarms if any
- Collection of required drawings, company policies, risk assessments, training records, SOPs, Safe Systems of Work, Permit-to-Work, Maintenance Records —— need and importance of all this information

- Utilize the knowledge and skills gained during training and write an Accident Investigation Report
